About Our Office

Office Hours

A receptionist is available Monday through Thursday from 9am-5pm and Friday
9am-1pm. Individual doctors may have appointments scheduled outside of
these times.

Appointments

We see patients by appointment only. Evening hours are
available. If you would like to schedule an initial consultation appointment, 
please call the office at 586/776-3366 and follow the prompts for new patients. During normal business hours you may also press Extension 3. In addition, you may also contact our office via our website from the home page. Our staff will assist you in scheduling an appointment if you were
referred to a specific doctor, or help you to determine who would best meet
your individual needs.

Prior to your first appointment new patient paperwork will be available to you through our secure Patient Portal, or you may download and complete the
appropriate New Patient forms located under the New Patient tab above. You
may also complete the appropriate social-medical history form regarding the
patient.

Payment and Insurance

We participate with many types of insurance plans, including Blue Cross and
Blue Shield, both traditional and PPO plans. However, insurance plans vary
greatly. For that reason, if you wish to use your insurance benefits, we ask
that you be aware of what your insurance plan covers, particularly
deductibles and co-pays. Deductibles are an amount set by your plan that
you are first required to pay out of pocket before your insurance will provide
any benefits. A co-pay is an amount that you are required to pay once your
deductible, if one exists, has been met. Please be aware that mental health
benefits are often different than medical benefits.

We do not participate with Carelon (Beacon Health), Cigna, Medicaid, Total
Health Care or United Behavioral Health (United Health Care) Plans.

The psychiatrists, Drs. Lujan and Mazzara, do not participate with Blue Care Network or HAP.


If you do not have any mental health benefits, or are responsible for a
co-pay, or if we do not participate with your carrier, we request that payment
is made at the time of service unless other arrangements have been made
with your individual doctor. Please note we cannot accept Medicaid patients at this time.


Please be sure to bring your insurance card with you to your appointment, 
and notify us if your insurance coverage changes.

Notice for Patients Over 18 Years-Old

Due to health privacy laws (HIPPA), if you are 18 or over we are unable to
speak to anyone other than yourself regarding bills or treatment (this
includes parents or spouses who may carry the insurance or make payments
on your behalf.) If you want to allow us to speak with someone other than
yourself, please sign a release of information. Please ask the receptionist if
you wish to complete the release or have any questions.

Patient-Centered Medical Home

The patient-centered medical home model is a home base for your health
care needs and a partnership between you, your doctor and other office
team members/specialists. The care you receive is centered on you. We
pledge to communicate with your primary care provider about your treatment
and progress so that we may all work together to provide you with the best
possible care. If you would like more information, please ask our staff for a
copy of the Patient-Provider Partnership Agreement.

Confidentiality and Electronic Transmissions

Every effort is made to maintain privacy and confidentiality of patients. 
Please be aware that the use of email and/or text to communicate with this
office or your therapist can compromise this confidentiality. As with all
personal health information, take caution when sharing information via these
mediums.

Our office is located at 19900 East Ten Mile Road, one block East of the I-94
expressway. We are located in the same building as Keller-Williams Realty.
